Request for Removal Form

Pursuant to Florida Statute 98.045, once a voter is registered, the name of that voter may not be removed from the statewide voter registration system, except at the written request of the voter, by reason of the voter’s conviction of a felony or adjudication as mentally incapacitated with respect to voting, by death of the voter, or pursuant to a registration list maintenance activity conducted pursuant to s. 98.065 or s. 98.075.

To have your name removed as a registered voter from the Nassau County, Florida rolls, you can print out the Request to RemoveOnce printed, complete the form, sign, and mail it to Nassau County Supervisor of Elections at 96135 Nassau Place, Ste. 3, Yulee, FL 32097, or you may hand deliver it to either of our offices.

Under Florida law, e-mail addresses are public records. If you do not want your e-mail address released in response to a public records request, do not send electronic mail to this entity. Instead, contact this office by phone or in writing.
