VOTE BY MAIL (Absentee) REQUEST FORM
Any registered Nassau County voter is entitled to a Vote by Mail ballot. A Vote By Mail (Absentee) Ballot may be requested for a specific election or for all elections through two regularly scheduled general elections. A Vote by Mail ballot may be requested in person at either of our offices, in writing, or over the telephone.

The voter requesting a Vote by Mail ballot must provide the following information:
If a member of the voter’s immediate family* is requesting the Vote by Mail ballot, the requester must provide the voter’s information listed above, as well as the following:
*Immediate family is defined as: spouse, parent, child, grandparent or sibling of the voter or of the voter's spouse.

A designee may pick up a Vote by Mail ballot for a voter on Election Day or five (5) days before Election Day. Designee must provide:

PLEASE NOTE: Florida Statute 101.62(4)(b)(4) limits pick up of Vote by Mail ballots by a designee to two (2) ballots per election, other than their own or immediate family.
Specific instructions are provided on the actual Vote by Mail ballot, and the secrecy sleeve enclosed in the Vote by Mail kit mailed to you. It is imperative that you follow the instructions very carefully. The voter must personally vote the ballot, unless assistance is required due to disability.
In order for your ballot to be counted, your Voter’s Certificate Envelope must contain your signature. Florida Statutes requires the County Canvassing Board reject as illegal any Vote by Mail ballot that is not signed.
Your voted and signed Vote by Mail ballot must be received by the Office of the Supervisor of Elections by 7:00 p.m. on Election Day in order to be counted.
